Equation of plane passing through point x',y',z' and direction ratio of normal is a,b,c is a(x-x') +b(y-y')+c(z-z')=0 Here x',y',z' =1,2,1 and what is normal Normal is a line perpendicular to given plane So direction ratio of normal will be (2-1),(3-4),(5-2) =1,-1,3 now put the values in given eqn and proceed.
