Dr. Sadao and his servants were in a dilemma. Dr Sadao didn't want to help the prisoner of war but as a doctor it was his responsibility to save the life of the patient who was in front of him dying. It was his responsibility to be loyal towards his ethics as well as to be loyal towards his country. Similarly, the servants were also in a dailyma of loyalty towards their master and their country. Consequently, they were in dilemma of the loyalty towards their country and their loyalty towards humanity.
