Dr sadao's decision was to save the American soldier even though he knew what his country has caused japan ....dr. sadao also knew the consequences of saving him but he reminds himself what was his duty at that moment he reminds himself of the the oath that he had took in order to save evey person that he could without discrimination on any basis.At the end he made sure tgat that the American soldier flea away from japan eaisly .
