Here the writer wants to convey that only the sufferer knows how much pain he is suffering from as in the fiction letter the coachman Ali only knew how he was feeling when he didn't get any letter from his daughter for whole 5 years.Now you can expand this and sorry if there are any grammatical mistakes?

It simy means that person knows what is pain who bears it .like in that ch !! Write urself ...humlog wala ans kaam nhi aayega
